Before your first appointment, please do the following:

Print out all forms under the forms tab

Note: It is not necessary to print all the pages of the Policy and Privacy documents, only the signature pages.

Please return all completed forms either as email attachments or by fax: 206.965.9191

It will take 20-30 minutes to complete forms.

It is important for all of this paperwork to be completed and returned at least one hour before the start of the first appointment. The appointment will have to be rescheduled if I don't have access to this information before the appointment time.

For paperwork to be complete I must also have photos of the front and back of your insurance card.

All appointments will be conducted via Telehealth

You will receive a link for the Telehealth portal via email.

If you currently take any prescription medications

Please provide a detailed list of any medications you are currently taking for any reason, including names and dosages.
